Model Release Form

Been meaning to do one for a while now and after realising that I need to start thinking about my work professionally I decided to create a model release form. I've done a few portraits in the past for either previous college projects or for personal work and by getting my models to sign my release form I will have full control over what happens to my images. It also means I can have a track record
of when the photoshoot had taken place and with whom I took the shoot with, aswell as recording the models contact details.
I got the form template from a website and modified it to suit me, including my name and website at the top of the page. I'm still planning on making a small logo
for myself and will add this to the form when i have completed it.
